Scope of work 

The Nurse Navigator will assess the patient's individual needs and develop a plan of care to meet those needs. This role includes providing education and support to patients, families, staff and caregivers. The goals of this position include improving patient care and flow, eliminating barriers, and improving the overall patient experience by connecting the patient and family with appropriate resources and services. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post-acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as a leader and resource for the health care team. Integrates new knowledge and evidence-based research into clinical practice. This position reports to the Clinical Nurse Manager and/or the Director of Nursing.

Qualifications

  • Required Bachelor's Degree in Nursing
  • 3 years of relevant experience nursing experience Required
  • LIC-Registered Nurse (RN) - STATE_MI State of Michigan Upon Hire required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association Upon Hire required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross Upon Hire required
  • CRT-At least one License and/or Certification in area of specialty - UNKNOWN Unknown preferred
