Overview

As a Clinical Field Manager, you empower a team of home care providers, ensuring every aspect of patient care is delivered with precision and adherence to VNS Health’s esteemed policies and industry standards. You will be responsible for leading, coordinating, and overseeing a diverse team of dedicated interdisciplinary professionals, from early career to seasoned clinicians. As an integral part of the clinical management team, you uphold the mission, vision, and values of VNS Health, supporting a team that elevates patient-centered care to New Yorkers most in need. Moreover, you provide unwavering support and guidance ensuring their seamless integration into VNS Health Home Care.

 What You Will Do 

  • Oversee an interdisciplinary team of community-based clinicians that deliver quality patient care from admission to discharge.

  • Manage, empower, and motivate team members in a dynamic work environment.

  • Proactively execute administrative functions to achieve seamless patient care including the review of clinician caseloads, staffing needs, recruitment process, and assigned scheduling of team members.

  • Use and learn tools and systems, problem solve new and different situations, and escalate issues timely to leadership to achieve organizational success and improved clinical outcomes.  

Qualifications

Licenses and Certifications:

  • License and current registration to practice as a Registered Professional Nurse or Physical Therapist in New York State, required

 

Education:

  • Bachelor’s degree in nursing or physical therapy from a program approved by the New York State Department of Education or equivalent work experience or

  • Master's degree in nursing or physical therapy or equivalent work experience, required

Work Experience:

  • Minimum of three years nursing experience and one year home health experience or

  • Minimum of three years of physical therapy experience, required

  • Clinical management experience, preferred
