Summary of Job Duties:

The Community Program Manager will provide case management services to current and recently discharged patients with a violent assault injury in the UAMS ED and Trauma Center as part of a locally funded hospital-based violence intervention program seeking to link individuals to mental, behavioral, and social services post discharge. The Program Manager will provide oversight/supervision of project staff related to facilitation of case management services.


Qualifications:

  • Master’s degree plus two (2) years of experience working with social services, health care systems, victims’ services, mental health or substance use treatment programs or community-based programs/organizations

  • Bachelor’s degree plus four (4) years of experience working with social services, health care systems, victims’ services, mental health or substance use treatment programs or community-based programs/organizations.

  • High school diploma/GED plus eight (8) years of experience working with social services, health care systems, victims’ services, mental health or substance use treatment programs or community-based programs/organizations required.

Knowledge Skills & Abilities:

  • Must have previous experience of providing case management services to vulnerable populations in order to ensure the ability to build rapport and provide support to the program participants.

  • Experience with facilitating collaborative partnerships, client assessment, treatment or action planning, conducting interviews, advising/counseling/mentoring clients, and participating in health promotion and health education activities.

  • Intimate understanding of and experience working with the community served by the individuals impacted by community violence.

  • Valid driver’s license and excellent driving record with reliable car and current automobile insurance

  • Ability to initiate and maintain positive working relationships with project staff and other organizations.

  • Ability and willingness to provide emotional support, encouragement and motivation to program participants

Preferred Qualifications:

  • Experience engaging communities with high prevalence of violent crime in Little Rock.

  • Experience working with community-based organizations that provide mental and/or social services to individuals impacted by community violence.

  • Experience with community violence prevention programs

  • Experience living or working in Little Rock, AR.

  • Experience working with diverse groups of people.

  • Experience working as part of a multi- disciplinary team, collaboratively practicing with health professionals, governmental, and/or community-based organizations.

  • Experience using creative and interactive approaches to engaging and building rapport with clients/participants.

  • Experience with case management, and service referral and navigation.


Additional Information:

Responsibilities:

  • Operate in a supportive role within an interdisciplinary research team.

  • Provide case management and navigational services to current and recently discharged patients with violent assault injury.

  • Conduct needs assessments; develop and maintain follow-up action plans; and facilitate and track service referrals for program participants.

  • Collaborate with Emergency Medicine, Trauma, and Psychiatry healthcare providers, social workers, and community-based organizations to provide follow up health care and social services that will maximize patient health outcomes.

  • Works under the direct supervision of the Program and Clinical Directors.

  • Assistance with appointment scheduling is a requirement of the job. Specific training in violence prevention services will be provided.

  • Provides empowerment, education, and case management services to current and recently discharged patients treated at UAMS for a violent assault.

  • Conducts periodic assessments of patients’ social, medical and other healthcare needs, and social and economic resources.

  • Provides consistent and frequent engagement with participants.

  • Conducts home visits when appropriate and necessary.

  • Educates patients about the health care system and facilitates relationship building between the two.

  • Documents work with patients through appropriate record keeping that follows the project’s policies and procedures.

  • Makes referrals for needed services and facilitates access to community resources.

  • Participates regularly in project team meetings.

  • Participates/assists as needed in any activities related to the program.

  • Works in collaboration with partners of the study and attend necessary ongoing training.

  • Advises participants and others regarding health care and other facilities available to them.

  • Assists participants in utilizing such services.

  • Makes follow-up contacts when required.

  • Serves as liaison between the professional staff and the community, including developing relationships with various stakeholders in the social service community.    

  • Provide oversight of the daily operations of the HVIP service project, including the supervision of staff and coordination of all case management related activities.

  • General oversight of program goals, products, and deliverables as it relates to participant retention and case management.

  • Develop best practices for case management and tracking and adopt these practices to be implemented as part of the standard operating procedures.

  • Develop and conduct regular meetings, trainings, and workshops for staff to successfully execute case management/linkage intervention components.

  • Coordinate follow up and monitoring of action items from meetings.

  • Responsible for the oversight of all linkage/case management activities, including monitoring the fidelity of intervention delivery.

  • Oversee the implementation of linkage components and related data tracking activities of staff, monitors progress, develop a scheduling and monitoring system with staff, help to problem-solve, and assist staff when needed.

  • Assists in gathering and evaluating data concerning the program; may perform incidental clerical duties such as keeping records, answering the telephone, arranging participant appointments, requesting medical records, etc.

  • Provide regular oral and written progress reports to the Directors and team.

  • May provide other duties as assigned.
