You will provide specialist aged mental health support to residents across Berengarra Residential Facility, the Ellerslie Unit at St George's, and Prague House. Working within a Consultation Liaison Psychiatry model alongside the Visiting Psychiatrist and multidisciplinary teams, you deliver comprehensive mental health assessment, intervention, consultation, education, and support to residents, families, and staff. You play a key role in improving access to specialist mental health services, promoting recovery-oriented care, and enhancing the capability of residential aged care teams.

• Conduct comprehensive mental health and risk assessments, develop care plans, and deliver evidence-based psychological interventions for older adults in residential aged care.
• Triage, assess, and manage referrals through the APATT intake process, coordinating referrals and transitions to APATT, the Residential Support Program (RSP), and other specialist services as required.
• Collaborate with the Visiting Psychiatrist and multidisciplinary teams to deliver coordinated, recovery-oriented care and optimise consumer outcomes.
• Monitor psychotropic medication effectiveness and side effects, support safe medication management practices, and contribute to psychiatric reviews and Mental Health Tribunal/Community Treatment Order processes where required.
• Maintain accurate clinical documentation, activity reporting, waitlists, and referral processes using AutumnCare, MRO, and other organisational systems.
• Provide education, behavioural support, consultation, and psychoeducation to residential staff, consumers, families, and carers to build capability and improve mental health outcomes.
• Promote awareness, accessibility, and engagement with specialist aged mental health services across residential aged care settings while supporting timely intervention and continuity of care.

Classifications for this position will range from RPN Grade 4 NP75 - NP77 ($121,274 - $130,603 per annum pro rata)

Your Contribution

• Degree in Nursing (or equivalent) meeting requirements for registration as a Registered Nurse in Australia
• Relevant postgraduate qualifications in Mental Health Nursing or related discipline
• Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
• Minimum 5 years post-qualification clinical experience within Mental Health, including older adult mental health, case management, and care coordination
• Demonstrated commitment to evidence-based practice and excellence in clinical care
• Sound knowledge of the Victorian Mental Health and Wellbeing Act 2022 and related legislation
• Highly developed interpersonal, communication, and stakeholder engagement skills
• A valid Nationally Coordinated Criminal History Check, or willing to obtain
