JOB DESCRIPTION:

SUMMARY OF POSITION

Provides case management services to children and families in their home, in the community, and in school settings. Coordinates client care and maintains essential elements of the client’s record. Teaches and trains clients in activities of daily living, in a group and individual settings. Connects client with community support systems. Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Coordinates client care by working closely with a multidisciplinary team to develop and maintain essential elements of the client’s clinical record, including but not limited to treatment plan, progress notes, reporting and correspondence with external agents, and remain in compliance standards.
  • Teaches and trains clients in activities of daily living, in a group and individual settings, complying with clinical documentation requirements, and provides services according to organizational guidelines or protocols.
  • After evaluation and assessment, connects client with community support systems as determined to be appropriate, and maintains regular communications with schools, OFC’s and other family service providers that the client and their family is involved with or receiving services from.
  • Consistently exercises discretion and judgment to analyze, interpret, make decisions and decide what actions are necessary based on the varying facts and circumstances of each individual case.
  • Provides telephone and on-site crisis intervention/prevention and follow-up to clients and their families.

QUALIFICATIONS

Education Level

Bachelor’s degree.

Years of Experience

Three (3) years of supervised paid casework

experience, OR One (1) year of supervised paid casework experience in a licensed child placing agency or with the department, OR One (1) year of graduate training in a recognized school of social work. LCPA experience preferred.

$16.50--$23.10
