Position Summary

This is a full-time remote role requiring 50-75% travel around Long Island, NY (primarily Nassau County). May require visiting beneficiary in their home, SNF, hospital or physician’s office. Role may also include making outreach to beneficiaries and scheduling appointments from the physician’s office.

Working schedule is Monday-Friday; will work between 8:00am-5:00pm EST (Flex Start 9:00AM-6:00PM EST).

CVS Accountable Care Community Health Care Workers (CHW) serve the community as a bridge between the beneficiary (community) and the healthcare system (providers) through outreach and education. By providing these services, CHW’s help CVS Accountable Care beneficiaries attain and maintain better health outcomes, remove barriers to care, link with community resources, improve relationships with healthcare providers and help both parties to become more acculturated with each other. This position will work under the direction of a designated professional.

Fundamental Components & Physical Requirements include but are not limited to:

  • Requires flexibility to work in situations in which the working conditions are unknown, such as environments and situations of beneficiary’s homes and communities vary.
  • Ability to communicate medical information to health care professionals and care coordinators over the telephone.
  • Knowledge of formal and informal community agencies and resources, or the knowledge of how to research.
  • Working knowledge of multi- system outreach programs relate to health care delivery, clinical education, and health-related services.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. Educate healthcare professionals and providers on opportunities for improving and understanding the social determinants of health that may be impacting beneficiary’s health and treatment plan through various communication channels.
  • Occasional weekend hours may be required.
  • Requires the ability to travel to multiple facilities, offices, and home locations.
  • Able to influence medical care providers as needed to adjust interventions based on beneficiary’s cultural, ethnic, or religious preferences.
  • Ability to work collaboratively in a team and manage multiple priorities, utilizes effective time management skills, and exercise sound judgement.
  • Exceptional organizational and interpersonal skills, with attention to detail required.
  • Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings.
  • Conduct home visits with CVS Accountable Care beneficiaries with chronic conditions to reinforce following the care plan, adjusting tasks to fit with the beneficiary’s culture, language, and religion, improve access to care, identify health care needs, and reduce unnecessary hospitalizations and emergency department visits .
  • Build and maintain positive working relationships with the CVS Accountable Care beneficiaries, providers, nurse case managers, agency representatives, supervisors and office staff, from diverse cultural and socio-economic backgrounds. Work to reduce cultural and socio-economic barriers between clients and institutions.
  • Assist beneficiaries to identify socio-economic issues that affect their overall health and develop health/social management plans and goals

Required Qualifications

  • 1+ year of experience working with patients/community members with clinical or behavioral health needs.
  • 1+ year of experience outreaching to community and helping to locate resources.
  • Willingness to travel 50-75% and work independently. Must reside on Long Island, NY, preferably in Nassau County near Malvern, Lake Success, East Hills, or Plainview areas.


Preferred Qualifications

  • Bilingual in Spanish and English preferred
  • Licensed Practical Nurse (LPN) preferred
  • Community Health Worker Certification or equivalent training and experience preferred.
  • 3-5 years relevant experience. Experience in serving the Medicaid or Medicare Populations in urban or rural environments with familiarity of local formal and informal resource networks preferred.
  • Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPAA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions.
  • Meet all requirements set forth by local state i.e. first aid, CPR, TB screen, etc.
  • Experience using a computer including the use of Excel and Electronic Health Records.


Education

  • Minimum High school diploma or G.E.D., prefer LPN
  • Completed and approved CHW/Promotora certification program preferred. Will train after hire.

Pay Range

The typical pay range for this role is:

$18.50 - $37.02

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
