Obtains patient information and vital signs, assists the physician with patient follow-up calls, screening and follow-up with patient test results; maintains medical record information; coordinates patient care information with other health care providers; and obtains prior authorizations. Assisting in phone triage, this position answers the phone and directs callers to the appropriate person/department; takes messages and sets appointments for the patients. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his/her area.

Pay Range:  $18.49 - $27.38/hour

Employment Type:  Part Time, 40 hours every 2 weeks

Schedule:  Monday - Thursday

Requirements:  Graduate of an accredited LPN program required, Michigan LPN license, BLS for the Healthcare Provider within 60 days of hire

PATIENT CARE:

  • Applies patient screening practices, obtains patient information and vital signs.
  • Recognizes and responds to patient emergencies.
  • Answers the phone promptly and courteously, accurately taking and prioritizing messages and assisting callers to obtain satisfactory responses to their requests.

PATIENT/OFFICE FLOW:

  • Participates in maintaining a confidential, safe, patient focused office.
  • Communicates well with clients, referral sources, families and external customers as well as with the multi-disciplinary team of providers.
  • Anticipates and provides support to the providers so that providers can focus on clinical productivity.
  • Assures that messages and mail are distributed in a timely/accurate manner.
  • Screens calls as appropriate and communicates with patients in a professional manner.
  • Assures confidential questionnaires are completed and all necessary forms are signed.

CLINICAL FUNCTIONS:

  • Performs all clinical functions as directed by the physician. This may include pro-time, blood tests, injections for appropriate diagnosis.

DOCUMENTATION:

  • Documents patient information in the medical record.
  • Assists with maintaining patient medication logs, complete prescription files and communication logs.
  • Assists the physicians with the chart closing process.
  • Files records in accessible, confidential and secure system.

QUALITY IMPROVEMENT:

  • Supports the quality improvement activities of the facility and responds appropriately to change.
  • Participates in learning experiences as appropriate.
  • Requests supervision when needed and utilizes supervision effectively
  • Ensures compliance with policy, procedure and regulatory requirements.
  • Maintains patient rights, confidentiality and ethical billing practices.
  • Identifies communication needs of significant referral sources and assures that they receive information in a timely manner.

STAFFING COORDINATION:

  • Develops and monitors clinical team daily activities
  • Maintains and completes clinical team work schedule
  • Works with Practice Manager to develop action plans and follow up evaluations of disciplinary occurrences
  • Provides input in performance evaluation on clinical team members to Practice Manager.

-Graduate of an accredited LPN program required

-Michigan LPN license required
-BLS for the Healthcare Provider by 60 days after hire

-1 year experience working in an outpatient clinic or medical office preferred
