Job Summary:
Coordinates comprehensive care for ER patients to ensure seamless, cost-effective, and high-quality care delivery. Leads a multidisciplinary team in providing quality care to a designated patient population and their families. Evaluates patients for appropriateness of care and medical necessity, assesses and coordinates efforts to meet resource management goals, enhances patient outcomes, and monitors the cost-effectiveness of care. Performs all duties in accordance with the medical center’s mission, vision, and purpose.
Education & Experience
Education and Experience Required:
One (1) year of nursing experience in an inpatient setting.
Certifications, Licenses, or Registration required:
Valid RN license
Preferred Qualifications:
RN case management certification preferred.

- Performs and documents comprehensive assessments for ER patients whose diagnosis, procedures, or psychosocial needs indicate they would benefit from case management, ensuring rapid identification of at-risk patients.
- Collaborates with patients, families, and the interdisciplinary team to develop and implement a plan of care appropriate to the urgent nature of the ER setting.
- Continuously reassesses and documents patients’ clinical and psychosocial needs, as well as the effectiveness of case management interventions.
- Facilitates timely access to appropriate resources and services, coordinating with hospital departments, community agencies, and social services to address patient needs efficiently.
- Coordinates and implements case management interventions to achieve goals within the limited time frame typical of the ER environment.
- Leads case management activities for specific ER patient populations, such as frequent utilizers, high-risk patients, or those with complex social needs.
- Monitors patient progress and collaborates with the clinical team to adjust the plan of care as the patient’s condition evolves during their ER visit.
- Evaluates the appropriateness and medical necessity of proposed care plans, ensuring alignment with hospital policies and best practices for acute care.
- Facilitates safe and timely transitions, including admission to inpatient units, transfer to other facilities, or discharge with appropriate outpatient follow-up.
- Collaborates closely with social workers, nurses, physicians, and other team members to address patients’ immediate and long-term needs, including psychosocial support, community resources, and follow-up care.
- Utilizes reports, data, and scheduling tools to evaluate performance measures, identify trends, and support quality improvement initiatives within the ER case management program.
