Position Highlights:

  • Position: General Surgery-Bariatrics - Advanced Practice Provider (APN/PA)
  • Hybrid Position: Work from home 4 days per week, Evanston Hospital 1 day per week; outpatient medical group
  • Full Time/Part Time: Full-Time, 40 hours per week
  • Hours: Monday-Friday between 8am-5pm
  • Required Travel: Swedish Hospital facilities

What you will need:

  • License: Advanced Practice Licensure (APN-CNP) or (PA-C) in the State of IL required; Federal DEA and Illinois Controlled Substance License required

  • Education: Completion of  accredited program from either advanced practice nurse graduate/doctoral level nursing program OR Physician Assistant program
  • Certification: BLS/CPR certification required
  • Experience: Prior APN/PA experience preferred

What you will do:

  • Function as an autonomous Obesity Medicine and Bariatric Surgery Advanced Practice Provider within The Department of Surgery
  • Independently operate a full-time outpatient-based clinic managing both non-surgical and surgical patient populations
  • Provide comprehensive obesity centric health assessments, differential diagnosis and metabolic diagnostic screening
  • Formulate individualized medical-surgical treatment plans in a shared-decision making model
  • Prescribe anti-obesity pharmacotherapy and emerging evidence-based therapies to manage obesity and its related co-morbidities
  • Provide patient education, motivation, respect and empathy to the highest standard
  • Work autonomously and within multi-disciplinary team as a resource and consultant
  • Attend and participate in department team meetings, case conferences and support groups
  • Identify opportunities for and implement program growth initiatives
  • Positively contribute to safety outcomes, quality initiatives and promote high quality patient experience
