As a Private Duty Registered Nurse Case Manager, you’ll contribute to our success in the following ways:

  • Assesses the health needs of individual clients by evaluating them to determine if a home care admission is appropriate, interviews the client and/or caregiver to determine expectations and needs, completes the admission assessment, and develops a personalized plan of care for each client, and works closely with the referral source to ensure a smooth transition to home care.
  • Develops the plan of care provided by the home health aide, RN, and LPN by main communication with the clients/caregivers and physicians to facilitate continuity of care, prioritizing care, identifying goals, providing appropriate teaching materials and information on community resources to clients/caregivers, and coordinating services on behalf of the client/caregiver to ensure continuity of care (i.e., equipment/supplies, pharmacy, medical services, transportation, etc.)
  • Implements the plan of care by working closely with the scheduler/s to assure appropriate caregivers are assigned, updates the plan of care based on assessments and supervisory visits, evaluates effectiveness of the plan of care at recertification, and as needed.
  • Provides supervision of Private Duty RN, LPN, and Home Health Aides to ensure quality and continuity of care and that documentation meets the patients plan of care and needs.
  • Reviews documentation of the supervised disciplines for appropriateness and completion compared to the plan of care.

What is Required?

  • Graduate of an accredited RN Program
  • 2+ years’ experience in a clinical care setting or home health environment preferred
  • Current state license as RN
  • Excellent communication skills
  • Dedication to patient care
  • Ability to work as part of inter-disciplinary team

You will report to the Branch Director or Clinical Manager.
