Pharmacists are responsible for knowing the purposes, effects, and side effects of various drugs. Caregivers in this role should be able to tell from looking at a patient's profile whether or not it is safe to combine new medications with the medications that the patient is already taking. Modern drugs have the potential to significantly improve patient's quality of life, however, they must be taken correctly, with the proper frequency, and in the appropriate dosage.
A caregiver in this position works an evening/night rotation from 10:00am – 6:30pm and 12:00pm – 8:30pm.
A caregiver who excels in this role will:
Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.
Ensure the appropriate selection, dosing, administration and dispensing of medication orders.
Verify medication orders and/or dispense medications accurately according to state and federal regulations.
Document services in a complete, accurate and timely manner.
Actively contribute to medication safety initiatives.
Ensure all pharmacy operations are in compliance with national accreditation standards.
Educate and train students, residents, other caregivers, and/or patients.
Provide cost-effective care.
Be accountable for drug use policy adherence.
Minimum qualifications for the ideal future caregiver include:
PharmD or Bachelor of Science in Pharmacy from an accredited college of pharmacy.
Current State licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
Individuals required to administer immunizations: current Basic Life Support (BLS) certification through the American Heart Association (AHA) and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire.
Individuals required to respond to adult or pediatric cardiopulmonary arrest events: current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification through the American Heart Association (AHA), or certification obtained within one year of hire.
Preferred qualifications for the ideal future caregiver include:
Board certification.
Completion of a post-graduate year 1 residency (PGY1), fellowship or relevant post-graduate pharmacy experience.
Clinical Pharmacists hired in a specialized pharmacy practice area: post-graduate year 2 residency (PCY2) or relevant experience.
