Core Responsibilities and Essential Functions:
Patient Assessment
- Collaborate with physicians to establish and evaluate treatment programs, care plans, goals, and discharge plans.
- Develop a care plan with input from the patient and/or family.
- Create realistic discharge plans that align with patient and family goals.
Treatment Planning and Provision of Care
- Implement the physical therapy treatment plan.
- Select appropriate treatment activities to advance patient goals.
- Revise goals and care plans with patient/family input.
- Provide ongoing patient and family education and training.
- Offer discharge instructions and referrals to community resources as needed.
- Serve as a referral source for rehabilitation services and equipment.
Documentation
- Document evaluations, care plans, and treatment sessions according to departmental guidelines.
- Complete and submit all required documentation, including family education and discharge information.
General Requirements

Professional Development

- Complete core and annual competencies as required.
- Maintain licensure through continuing education.
- Participate in the orientation and training of new employees.
- Fulfill all hospital and system mandatory requirements.
Required Minimum Education:
- Graduate from an accredited physical therapy program.
- Bachelor’s, Master’s, or Doctorate in Physical Therapy.
Required Minimum License(s) and Certification(s):
- Physical Therapist License.
- Basic Life Support (BLS) Certification.
- BLS Instructor Certification (if applicable).
- BLS Provisional Certification (if applicable).
Required Minimum Skills:
- Effective communication, customer service focus, team orientation, and basic computer skills.
