This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients. As a Virtual Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative remote monitoring solutions. You will be responsible for performing the initial patient visit, developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care.
