Pay Range

$37.50 - $56.25

What You’ll Do – Essential Functions

  • Identify patients who would benefit from Case Management through initial screening and assessment of discharge needs.
  • Collect and evaluate patient information via interviews, chart reviews, and care team discussions to determine necessary interventions.
  • Develop, implement, and monitor a Plan of Care in collaboration with nursing, physicians, multidisciplinary teams, and community partners.
  • Lead and coordinate the discharge planning process, ensuring needs are addressed and transitions are smooth and cost‑effective.
  • Participate in multidisciplinary rounds, driving patient progression and communicating discharge planning updates.
  • Document all Case Management activities in the Electronic Medical Record and communicate essential information to providers and payors.
  • Monitor the medical necessity of admissions and ongoing care; collaborate with Utilization Review to address level-of-care changes.
  • Identify system issues, trends, or barriers to care; propose solutions and participate in quality improvement efforts.
  • Apply age‑ and developmentally‑appropriate care planning and communication techniques based on population needs.
  • Guide patients and families through post‑discharge options based on payer guidelines, community resources, and clinical needs.
  • Maintain strong partnerships with physicians to support the organization’s mission and enhance patient outcomes.

What You’ll Bring – Education, Experience & Licensure

  • Graduation from an accredited School of Nursing; BSN required, MSN preferred.
  • Active RN license from the North Carolina Board of Nursing.
  • Minimum 2 years of relevant clinical experience; prior Case Management experience strongly preferred.
  • Basic Cardiac Life Support (BCLS) required.
  • Certification in Case Management highly recommended within 2 years of employment.
