As Branch Director, Home Health, you will lead both clinical and operational excellence for your branch—building high-performing teams, ensuring exceptional patient outcomes, and driving financial and quality performance. This role blends Branch Director leadership with hands-on clinical oversight traditionally performed by a Clinical Team Manager (CTM).

What You’ll Do

Branch Operations & Financial Leadership

  • Organize and direct the day-to-day operations of the branch.

  • Drive achievement of clinical, operational, and financial performance goals.

  • Oversee branch financial performance, staffing models, and productivity.

  • Ensure adequate staffing levels to meet patient care needs.

  • Implement strategic growth initiatives and market development plans.

  • Approve and submit time and attendance for payroll (visits, mileage, hours, rates, etc.).

Clinical Leadership & Care Coordination (CTM Scope)

  • Provide oversight of all patient care services and clinical staff (RNs, LPN/LVNs, Therapists, LMSWs, HHAs).

  • Ensure timely start of care and coordination of physician orders and care plans.

  • Review and approve plans of care and clinical documentation for quality and regulatory compliance.

  • Conduct and oversee OASIS reviews and clinical quality audits.

  • Coordinate case conferences for recertifications, discharges, and level-of-care decisions (e.g., Hospice, Rehab, ALF).

  • Remain available during operating hours to support clinical teams and patient care needs.

  • Provide coaching, training, and performance feedback to clinical team members.

  • Support field work as needed.

Compliance, Quality & Risk Management

  • Ensure compliance with Medicare, Medicaid, state licensure, and accreditation standards.

  • Maintain documentation accuracy, timeliness, and regulatory compliance.

  • Lead QAPI initiatives and quality improvement activities.

  • Identify and mitigate clinical, operational, and staffing risks.

  • Address patient and family concerns promptly and professionally.

Talent & Team Development

  • Lead hiring, onboarding, training, and performance management for branch staff.

  • Build engaged, high-performing clinical teams.

  • Foster a culture of accountability, collaboration, and patient-centered care.

  • Develop trusted relationships across interdisciplinary and leadership teams.

What We’re Looking For

Education & Licensure

  • Associate degree in Nursing, Physical Therapy, or related healthcare field required

  • Bachelor’s degree preferred

  • Current state license as RN or PT (per state/federal guidance)

Experience

  • 1+ year supervisory or administrative experience in home health or related healthcare setting required

  • 2+ years of home health experience preferred

Skills & Competencies

  • Advanced clinical judgment and leadership skills

  • Strong operational and financial management abilities

  • Experience with Homecare Homebase (HCHB), Workday, and MS Office

  • Knowledge of Medicare Conditions of Participation and state licensure requirements

  • Strong communication, coaching, and relationship-building skills

  • Ability to lead in a fast-paced, high-accountability environment
