Job Description:

The Practice: Independent and entrepreneurial-minded group practice of 1 physician and four advanced practice providers seeks an advanced practice provider to join an established primary care clinic. The current physician is internal and family medicine, and board certified in internal medicine. This position offers high income potential with full partnership options within three years. Work/Life balance is fundamental to all associates and physician in the group.

Our practice is a member of the independent practice association (IPA), PHP prime. PHP prime provides wrap-around services to help make our practice successful, including care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Position:

  • Performs a comprehensive patient health assessment, including health history, physical exams, and preventive screening.
  • Orders, administers, and analyzes diagnostic tests.
  • Performs advanced technical procedures.
  • Establishes systematic follow-up for evaluation of the plan of care and reassesses and modifies the plan, as necessary, including initiation of referrals and consultation with physicians and other health care providers as appropriate.
  • Maintains accurate, legible, and confidential records.
  • Trains patients and families on managing illnesses or injuries and disease prevention.
  • Develops and maintains a good working relationship with the healthcare team.

Qualifications (Education/Experience/Knowledge/Skills/Abilities):

  • Possess current, valid licensure as an Advanced Practice Nurse from the Colorado Board of Nursing OR valid licensure as a Physician Assistant from the Colorado Medical Board
  • 5+ years nursing experience required.
  • Valid Prescriptive Authority in the State of Colorado
  • Current Drug Enforcement Administration (DEA) certification

Job Schedule: Full Time

Shift: 8:00 AM to 7:00 pm

Workdays: 4 days per week; usually Monday through Thursday, or Tuesday to Friday, occasionally Saturday

Salary Range: $100,000 - $120,000 + Depending on the experience.
