Case Coordinator Job Responsibilities

Current:

1. Meet with each new patient at the beginning of treatment (strive for 1st treatment)

2. Make follow up phone calls as tasked by nursing staff or providers

3. Round in infusion area, daily

4. Review fall risk assessment, weights, anxiety/depression assessments and other notes in chart to identify patients who may benefit from a referral to P/T, nutrition, Ways to Wellness, palliative care, transportation services, DORS, Lifescapes, Hospice or other community resources. Highlighted services need to be completed by nursing staff, but can be initiated by case manager.

5. Review consults for head and neck cancers for referrals for swallow evaluations and speech therapy

6. Call patients following their first treatment to check status

7. Call patients taking oral chemotherapy, weekly for first 12 weeks and complete questionnaire

(QOPE requirement)

QOPE is a quality initiative, Gold Standard of Oncology. We were one of the first in the state of Illinois to hold certificate.

8. Call patients identified as needing additional support

9. Facilitate monthly support group

10. Meet with patients, as needed, to provide additional, in-person support

Upcoming:

1. While rounding in infusion area, complete palliative care questionnaire.  If any needs are identified, have patient meet with LMR.
