How will this role have an impact?
The Clinician Player Coach is an exciting opportunity for seasoned clinicians to grow into an expanded role, serving as a mentor and champion for Signify Health clinicians, as well as continuing meaningful work connecting with health plan members.
The Clinician Player Coach completes In Home Evaluations (IHEs) and other associated visits for up to 80% of their week, and 20% of their time serves as a peer mentor to other clinicians. The ideal candidate will have demonstrated high quality, efficient care with high member satisfaction scores to be considered for this role. This role is responsible for providing dedicated guidance to new hire clinicians during their first 90 days and offering 1:1 support to active clinicians. This role is the first line of support for non-urgent clinical inquiries, and if needed, escalates clinical and/or operational issues to Clinical Leaders and Operations Managers respectively. This role will also serve as an early adopter for new diagnostic and preventive services as well as clinical workflow changes.
This role will report to our Operations Manager with a dotted line to the Clinical Success team.
What will you do?
Complete In-home Visits
Perform IHEs, Focused Visits as well as Video Visits, exemplifying the highest standards of Signify Health key performance indicators and metrics (includes operational, clinical, and member experience KPIs).
Primary coverage areas include but are not limited to California.
Test new Diagnostic & Preventive Services (DPS) and clinical workflow changes and provide feedback.
Provide 40 hours of availability weekly, consistently providing 128+ capacity units per month, and reserving one assigned day/week for coaching and mentorship activities (may include ridealong and job shadow visits during field days as well)
Execute a strong visit completion rate, consistently pacing 88+ completed visits per month
New Clinician Mentorship
Provide dedicated guidance in the first 90 days to new hire clinicians to help them acclimate to Signify Health and navigate available resources.
Facilitate virtual and in-person shadowing opportunities for new clinicians to observe and learn.
Provide best practice tips to new clinicians on clinical workflows, DPS, etc.
Active Clinician Mentorship
Offer Group Support to active clinicians on new features/DPS/updates of a clinical nature as well as areas of opportunity if noted in prospective monitoring of a clinical nature through webinars and/or office hours
Offer real-time support to in-field clinicians, using a variety of company-identified outlets to facilitate prompt outreach
Serves as first line of response for clinical inquiries before escalating to Clinical Leader (CL)
Other role expectations and requirements:
Meets regularly with Clinical Leaders (CL/CNL) for support and training
Consistently engage with regular manager, team and clinical meetings. May attend special events, as needed
Available to provide mentoring and coaching support including rotating evenings and weekends, as assigned
Maintains training, affiliation, and compliance with Video Visit requirements, regular training required
Maintains training and complete all diagnostic and preventive service (DPS) products as directed (regular training required)
Maintains all required supplies and associated diagnostic equipment related to DPS offerings
Be flexible to travel in-region and locally, as dictated by business needs
Accommodate field observations and/or “ridealong” experiences, as requested (both during in-field days and dedicated support days, as needed)

We are looking for someone with:
Ability to:

Understanding of:
How to manage inquiries and escalate performance issues to leadership
How to uphold high-quality member care across teams
Experience with:
Completing IHEs successfully, as defined by Signify Health operational, clinical, and member experience KPIs
Mentoring teams and providing 1:1 professional and personal support
Signify IHE tools and processes
Developing and presenting group presentations
Required: Clinical background and State Licensure (e.g., MD/DO, NP/PA)
Preferred: Multi-state licensure
Preferred: 5-7 years of clinical experience, with a strong background in primary care, chronic disease management, or related field
Required: Minimum 6 months - 1+ year(s) of experience as a high-performing Signify Health Clinician
Required: Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies (including, but not limited to, iPad, medical supplies and scale)

The typical pay range for this role is:
$95,738.00 - $206,206.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
