Position Summary

OhioRISE Community Health Worker program facilitates CHW services that are utilized to support coordination of member care by the health plan, serving as a liaison between the member and the overall health system.  Community health workers have unique expertise in conducting outreach, making them well positioned to help engage and refer members to care coordination services at the health plan. As such, CHW outreach and services are offered to target unable to reach members in the OhioRISE health plan. 

CHWs will conduct outreach to increase care coordination engagement; identify and refer members to health care and community-based resources; and to complete the Child and Adolescent Needs and Strengths (CANS) assessment for eligibility and enrollment. 

Required Qualifications
- 1+ year experience working with Customer Service.

- Obtain CHW (Community Health Worker) certification within 1 year of hire (Company provided).

- Must be able to travel within Ohio.

- Microsoft Applications working with Excel.

Preferred Qualifications
- Healthcare or Medicare field preferred.

Education
High School Diploma or GED.

Pay Range

The typical pay range for this role is:

$18.50 - $31.72

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
