The Home Health Registered Nurse uses an individualized patient care plan to provide top quality care to all of our patients, keeping them Happy, Healthy and at Home.

RN PRN ..

As an RN, you will...

• Visit 3-7 patients on any given shift, driving to the patients home providing skilled nursing services.

• Primarily responsible for the initial assessment and case development of patients in all stages of life in their homes.

• Be in charge of assessing, interpreting, planning, implementing, documenting and evaluating patients according to the patient’s age and diagnosis.

• Must be able to check your email intermittently during the week to respond to documentation requests.

• Must be okay with traveling throughout the service area!

Skills/Experience needed...

• Admissions, Wound Care (with vac), IV Therapy, PICC Lines, Lab Draws, and Patient Education.

• Strong assessment, skilled nursing and documentation
