What You'll Do
Functions as a member of the multi-disciplinary health care team to assess, advocate, and place care for patients with palliative care needs. Responsible for completing assessment, coordinate needs/arrangement for patients, families, and significant others, providing information/education/linkage with appropriate resources and services.
Requirements
Board certified Hospice/Palliative Nurse (CHPN) or obtain 15 hrs of CEU's within 3 months. Thereafter maintain CHPN or obtain 15 hrs CEU's in Palliative care annually
Current Missouri Advanced Practice Nurse licensure as a Nurse Practitioner according to MO State Board of Nursing.
Current Missouri Registered Nurse license or current Registered Nurse license from a compact state. If a compact license is held, it must be in the nurse’s state of residence.
