Job Description:

Hospice RN Case Manager

Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Job Type: Full-Time | Field-Based
Territory: Oakland County

As a Hospice RN Case Manager, you’ll provide patient-centered care that brings comfort, dignity, and peace to individuals and their families—right in the place they call home. Your role allows patients to spend their final days surrounded by compassion, respect, and the people who matter most.

What You’ll Do

  • Provide skilled and compassionate nursing care in patients’ homes

  • Complete comprehensive assessments and create individualized care plans

  • Monitor patient conditions and respond quickly to changes

  • Partner with physicians and interdisciplinary team members to coordinate care

  • Educate patients and families on disease progression, medication, and comfort measures

  • Offer emotional and practical support to families through end-of-life care

  • Ensure compliance with physician orders, care plans, and regulatory standards

What You’ll Bring

  • Graduate of an accredited nursing program (Diploma, ASN, or BSN)

  • Current RN license in the State of Michigan

  • Minimum of 1 year of nursing experience (hospice or home health preferred)

  • Reliable transportation and ability to travel within Oakland County

  • Strong clinical judgment, communication, and organizational skills

  • Ability to lift up to 50 lbs and perform the physical requirements of home visits
