Job Summary:


The Patient Navigator is the 'front line' personnel who will act as a patient advocate working with the patient care team in
communicating scheduling issues. The position will assess the patient's needs, assist physicians and nurses with moving
patients through the continuum of care by scheduling patient appointments across several sub-specialties, making
appropriate referrals to provide navigation and guidance and navigate all criteria-based scheduling.

Core Responsibilities and Essential Functions:


Patient Care manages the patient's case and ensures the best path for care. The familiarity of communities
they serve the society according to age, race, language, ethnic and socioeconomic groups, and development
of trusting and therapeutic relationship for improved well-being. Promoting one-on-one contacts to aid
patients, families, and caregivers to get through multifaceted health networks.
Coordination of Care: Helps patient coordinate appointments across multidisciplinary care teams and refers
them to internal/external resources as needed. Acts as a liaison between patient, nurses, physicians, allied
health professionals, and others on the care team. Collaborates with team to develop a comprehensive care
plan based on the patients individual needs and preferences.
Patient Advocacy: Serves as the primary educational resource for patients and families on care provided.
Communicates relevant information to patients, family-member, physicians, and members of the patient care
team. Addresses patient concerns and questions they might have concerning care. Provides education
strategies to encourage a sense of empowerment.
Collaboration: Well-developed partnerships with community clinics and social service entities. Collaborates
with the primary care/speciality care providers to develop a comprehensive care plan based on the patient's
individual needs, preferences, and insurance permissibility. Responsible for continuous communication with
the referral coordinator and making sure all necessary paperwork is completed before the patient's scheduled
day.
Strong communication, interpersonal, assessment, teamwork, initiative, and versatility are critical skills for
the position. Expected personal characteristics of the Patient Navigator are language is spoken, commitment
to community, knowledge of cultural values and beliefs, relationships with community and stakeholders.
Performs other duties as assigned
Complies with all Wellstar Health System policies, standards of work, and code of conduct.

Required Minimum Education:

  • Associates Health Administration

Required Minimum License(s) and Certification(s):


All certifications are required upon hire unless otherwise stated.

    Required Minimum Experience:


    additional years of relevant work experience and/or other specialized training can be used in lieu of
    minimum education requirements. Required
    Minimum 2 years healthcare setting, patient scheduling or care coordination. Required
