Hybrid Opportunity for RNs living in Indiana, Illinois, Kansas or Missouri
The Transitional Care Manager is responsible for overseeing the overall transitional care duties, operations, goals, and outcomes of progress while acting as a liaison between service lines. The Transitional Care Manager assists all employees and clients as the care plan progresses through the continuum of care. This leadership position involves managing daily operations, coordinating patient care activities, and promoting a positive and collaborative work environment. This position is responsible for the oversight of the Transitional Liaison role that identifies potential end-of-life and alternative patient care needs for skilled home health patients, assesses patients for clinical appropriateness, hospice eligibility, Palliative Care consult opportunities, and serves as an educator to internal and external stakeholders regarding hospice benefits and the overall transition process between services. The Transitional Care Manager will work closely with service line leaders to enhance patient outcomes and maintain compliance with regulatory standards.
