Care Coordinator - RN - ED – 4x10 - 12pm - 10PM Tues - Fri - Weill Cornell

Across NewYork-Presbyterian/Weill Cornell Medical Center, Care Coordinators are the guardians of continuity of care; they are the skilled clinicians leading the full spectrum of patient care, from pre-admission to discharge while ensuring quality outcomes. As a Care Coordinator at NewYork-Presbyterian, you'll serve as a highly valued and respected leader from day one, empowered to make amazing things happen for our patients and their families.

Working in a fast paced ED trauma center, you will coordinate activities of the interdisciplinary team members focusing on effective management of care across the continuum from admission to discharge based on established LOS guidelines.

You'll manage complex cases and serve as a strong and knowledgeable resource to the interdisciplinary team for understanding the case management process . Direct all aspects of inpatient care, ensuring optimal length of stay. Anticipate outpatient service needs, setting appointments and consultations prior to discharge. 

This position will be from noon to 10 pm , 4 x 10 hour shifts Tuesday -Friday rotating Saturdays (1 time per month).

Preferred Criteria

  • Emergency Department experience preferred

  • 2 years' of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting, Emergency Department experience preferred

  • 1 year of leadership experience in a healthcare setting

  • Master's Degree in healthcare related field 

Required Criteria

  • Bachelors degree in Nursing (BSN)

  • Current NYS Registered Nurse (RN) licensure

  • 3 to 5 years of health care experience in an acute care setting

  • Superior communication, demonstrated problem solving skills, strong business acumen, and interpersonal skills are critical

$100,000-$140,000
