Job Description:

Scope of Position:  The Patient Navigator’s primary function is to guide patients with abnormal findings through the health care system by assisting with access issues, developing relationships with service providers, tracking interventions/outcomes, as well as assisting in the development of the patient navigation program that will result in timely resolution of abnormal findings.

                                                           

ESSENTIAL FUNCTIONS

  • Navigate cancer screening patients with abnormal findings through the health care system, responsible for providing follow-up and case management services to all patients with abnormal findings, while maintaining adequate documentation and records.    
  • Develop and maintain relationships and resources to be used in patient navigation program, including relationships with providers and community resources, tracking systems, and policies/procedures.
  • Reporting and evaluation of patient navigation program including but not limited the breast and cervical programs for both Louisiana and Mississippi
  • Stay updated on current cancer screening guidelines, prevention and risk for the 5 types of cancer that are screened.  Must  have 4 hours of continuing education.
  • Performs other duties as needed.

QUALIFICATIONS:

  • A college degree in nursing desired. 
  • LPN/RN
  • Healthcare experience desired.
  • Familiarity with Microsoft Word, Excel, Access, and PowerPoint.
