Job Description:

Territory: Chicopee, Holyoke, Springfield
Schedule: Monday–Friday, Days

Provide compassionate, holistic care for behavioral health patients in their home setting
Assess and manage a caseload to identify physical, psychosocial, and environmental needs
Develop and adjust individualized care plans in collaboration with physicians and interdisciplinary team members
Educate patients, families, and facility staff on behavioral health and symptom management
Supervise and support LPNs and behavioral health aides to ensure quality of care
Current, unrestricted RN license in the state of practice
Minimum 1 year of clinical nursing experience
Reliable vehicle, valid driver’s license, and auto insurance
Willingness to travel within assigned territory (approx. 50%)
Associate or Bachelor’s degree in Nursing (preferred)
Home Health or Hospice experience a plus
Experience with Homecare HomeBase preferred
