🩺 Behavioral Health Registered Nurse – Home Care

$5,000 Sign-On Bonus!
Territory: Chicopee, Holyoke, Springfield
Schedule: Monday–Friday, Days

🩺 What You’ll Do

• Provide compassionate, holistic care for behavioral health patients in their home setting

• Assess and manage a caseload to identify physical, psychosocial, and environmental needs

• Develop and adjust individualized care plans in collaboration with physicians and interdisciplinary team members

• Educate patients, families, and facility staff on behavioral health and symptom management

• Supervise and support LPNs and behavioral health aides to ensure quality of care

✅ What You’ll Need

• Current, unrestricted RN license in the state of practice

• Minimum 1 year of clinical nursing experience

• Reliable vehicle, valid driver’s license, and auto insurance

• Willingness to travel within assigned territory (approx. 50%)

• Associate or Bachelor’s degree in Nursing (preferred)

• Home Health or Hospice experience a plus

• Experience with Homecare HomeBase preferred
