Job Description Summary:

Under the general supervision of the Director of Clinical Services, responsible for assessing, recognizing a patient's worsening condition, intervening quickly, and directing team members in order to prevent or halt a change in patient's status that may lead to a medical crisis using approved protocols on all units. Employee is subject to call back and overtime as required by the hospital.

ESSENTIAL JOB FUNCTIONS:

  • Rapidly responds to Medical Response Team calls, assesses the patient in need, collects history, and quickly implements approved protocols according to hospital policy.

  • Coordinates or delegates tasks to team members as needed during the event.

  • Coordinates the notification of the primary physician or on call physician using the S.B.A.R. format.

  • Stays with the patient until the crisis is resolved, including any diagnostic procedures or transfer to a different unit as required.

  • Serves as a mentor and resource to the nurse who has been caring for the patient.

  • Documents in electronic medical record.

  • Documents on the MRT flowsheet.

  • Communicates with patients and their families and collaborates with other disciplines for providing emotional support and teaching as needed.

  • Performs follow-up visits at 12 and 24 hour post event and documents findings on MRT follow-up sheet.

  • Communicates information regarding MRT follow-up needs to other shifts and managers and uses proper lines of communication to other departments of the hospital.

  • Responds to Rapid Response Team calls and Code Blue calls secondary to MRT calls.

  • Inserts Vascular Access Devices which include Peripheral Inserted Central Catheters (PICC Lines), Midlines, and IV’s. Documents all line insertions in electronic medical record.

  • Ensures the proper segregation and safe disposal of hazardous pharmaceutical waste.

  • Serves as a clinical resource throughout the building as needed by the Administrative Nursing Supervisors.

  • Assists in orientation of new personnel to the Clinical Services department as needed and serves as role model as required.

  • Assumes responsibility for continued professional growth.

  • Contributes to a positive work climate and the overall team effort of the department and hospital and greets guests in accordance to our Guest Excellence principles.

  • Performs related responsibilities as required or directed.

JOB SPECIFICATIONS:

EDUCATION:

  • Completion of a diploma, associate, or baccalaureate degree in nursing.

LICENSURE, REGISTRATION, CERTIFICATION:

  • Current licensure as a Registered Nurse as approved by the Tennessee Board of Nursing for practice in the state of Tennessee.

  • BLS, ACLS, and PALS certifications required within the orientation period.

  • NIHSS required.

  • CCRN desirable.

EXPERIENCE:

  • At least 2 years of current Critical Care experience required.

KNOWLEDGE, SKILLS AND ABILITIES:

Skills and proficiency in the principles, concepts, and techniques of the nursing profession as acquired through completion of a diploma, associate, or baccalaureate degree in nursing.  Knowledge of policies and procedures of the hospital and completion of skills check sheet for designated areas as acquired through 3 month probationary period.
