Job Description Summary:

This position is a very unique role that requires many skills especially triaging skills and computer navigating skills to be successful. Strongly recommend a period of shadowing of 3-4 hours to understand the role and responsibilities. Basic description of what a Transfer Center Nurse does: 24,7 department that requires weekends and holiday rotations Receive phone calls from outlying facilities to triage patients regarding transfers to one of our OhioHealth facilities (manage inbound transfers to 8 OhioHealth facilities and growing). Responsible for bringing in patients from outlying facilities who require a higher level of care or specialties that are not offered with outlying hospital. Obtain full report on patients and determine what services to call in regards to the patient's current needs and level of care. Ensuring reports/information get to the appropriate unit, physician, and sub-specialties. Communicate with all medical professionals (physicians, nursing supervisors, nurses, physician assistants, nurse practitioners, transport, charge nurses, sub-specialties etc. ) in regards to a patient's transfer to ensure quality and efficient care. Requesting a demographic sheet on the patient whom is being transferred from outlying hospital for pre-admission process at receiving hospital. Help arrange transport for patient by air or ground route per outlying facility request. Multi-tasking several cases at a time while providing quick acceptance of the patient to ensure care is not halted and transport of patient is efficient. Implementing multiple processes for acute patients: Stroke Alerts, STEMI Alerts, Trauma Alerts, Cardiac Arrests, Critical limbs, OB/GYN emergencies, etc. Following multiple processes for all our campuses within OhioHealth.

Responsibilities And Duties:

Maintains flow of patients throughout the hospital to include: emergency department, operating rooms, invasive recovery, cath labs, and nursing units.

Assesses Hospital admission to ensure appropriate admissions, level of care, medical necessity and utilization of resources. 1. Telephones triage and reviews documentation to assure outlying hospital direct admissions/ED transfers receive appropriate and timely bed management 2. Initiates clinical pathways on emergency and direct admissions 3. Screens observation admissions for appropriateness with approved criteria and directs to most optimal level of care. 4. Contacts physicians regarding order clarification and change in appropriateness for level of care needs. 6. Activates patients to assigned unit and bed. Assures correct assignment of status.

Plans and coordinates safe transfer of inpatient patients to accommodate needs of each admission. Works with nursing units to identify patients to transfer to lower levels of care to accommodate patients requiring higher acuity.

Collaborates with nursing administration to assure all resources are allocated to best meet the needs of organization.

Evaluates each patient continuously to identify change in required level of care to assure appropriate placement GMB versus Tele or telemetry versus intermediate or critical care versus telemetry, workup issues and admissions versus discharges.

Provides superior customer service to meet needs of patients, families, physicians, outlying hospitals, physician offices, nursing facilities, urgent cares, home care, hospice, and other telephone contacts received by department.

Provides education and support for bed management practice to interns, residents, and attending physicians.

Provides resources for organization regarding a variety of topics including: policy/procedure, department location, crisis management, staffing, department responsibilities and various other topics.

Identifies opportunities for process improvement and provides ongoing feedback.

Acts as liaison between all central Ohio hospitals to identify bed availability within Central Ohio area in order to assure timely and accurate placement of patients.

RN - Registered Nurse - Ohio Board of Nursing

Additional Job Description:

Effective verbal and written communication, problem solving and organizational skills; computer skills (CareConnect); typing skills, Clinical pathway development and understanding of quality improvement. 3 - 5 yrs. in emergency department/critical care; previous leadership roles (charge, preceptor).
