JOB DESCRIPTION:

SUMMARY OF POSITION

Partners with youth and families utilizing the High-Fidelity Wraparound Process to assist in building informal support networks through the use of formal team-based services to assist youth who are at risk for out of home placement or have ongoing mental health difficulties in order to empower youth and their families. 

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Help youth and family create plans that outline strategies to address family needs using a strengths-based approach.

  • Assist youth and team members in identifying and leveraging resources.
  • Provide support, training and assistance to other community members working with youth and family.

KNOWLEDGE, SKILLS & ABILITIES

  • Effectively communicate via written, verbal, in person and virtual methods.
  • Excellent organizational skills.
  • Strong computer skills.

QUALIFICATIONS

Education Level

Bachelor's degree

Years of Experience

Minimum two years of qualifying experience with SED (seriously emotionally disturbed) population with experience in case management, therapy, or skills training in conjunction with a mental health center, or other work that is targeted to a defined SED population and not incidental contact within another position.

Experience using electronic health records and/or online assessment databases.

Certification/Licensure

Obtain and maintain state certification as a wraparound facilitator.

Pay Range:

$16.50--$23.10
