Summary :

The Social Worker MSW provides quality psychosocial care to patients and coordinates care with the medical team while consistently applying the social work code of ethics. Through the coordination of care, conducts a thorough assessment of the patient’s needs. Understands the medical plan of care, coping abilities, and barriers to care. Implements interventions and/or provides resources.

Job Description:

KEY ACCOUNTABILITIES:

  • Identifies patients with complex care needs, conducts patient psychosocial status assessment, establishes a rapport with patient/family, and actively participates in creating and implementing a plan of care to achieve outcomes.

  • Utilizes effective oral and written communication skills to collaborate with the health care team as needed to enhance quality and care.

  • Develops psychosocial plan of intervention for the patient and family utilizing resources within the community. Incorporates social, emotional, financial, and spiritual aspects of care into the patient’s plan.

  • Evaluates progress toward outcomes/goals for individual plan for patient and modifies plan as appropriate.

  • Serves as a consultant, patient advocate, and liaison for patients and families in areas of social work needs and complex psychosocial issues that could include facilitation of specialty-specific psychoeducational support groups in some care settings.

  • Arranges for resources/funds to obtain medications, DME, housing, transportation, and other needed services. Facilitates appropriate and prompt access to primary care providers, physician specialists, and similar services as necessary.

  • Coordinates patient information and communication between and among the patient/family and the referring/accepting facilities, and communicates to caregivers as applicable to ensure smooth transition of care.

  • Advocates for the patient and performs discharge planning/after care coordination. Supports efforts to decrease hospital readmissions when possible and returns patient to the community with appropriate resources.

  • Facilitates advanced care planning and lifespan planning.

QUALIFICATIONS:

  • Master’s degree in social work

  • Licensed/Certified as Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Independent Social Worker (CISW), or Certified Social Worker (CSW)

  • American Heart Association Healthcare Provider Basic Life Support (BLS)

PHYSICAL DEMANDS:

  • Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance

  • Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

WORK ENVIRONMENT:

  • Frequent exposure to sharp objects and instruments

  • Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock

  • Occasional high noise level in work environment

  • Possible exposure to communicable diseases, hazardous materials, and pharmacological agents

  • Occasional contact with aggressive and or combative patients

Hospice Case Managers are responsible for overseeing the care of patients who are nearing the end of their lives. They work with a team of medical professionals to ensure that these individuals receive high-quality palliative care, including physical, emotional, and spiritual support.

Hospice Case Managers have a wide range of responsibilities, which can include:

  • Meeting regularly with patients and their families to monitor the patients’ physical, emotional, and spiritual needs.
  • Assessing patients’ social and medical history to develop plans of care that meet their needs.
  • Working with physicians, nurses, facilities and other medical professionals to coordinate care and treatment plans for each patient.
  • Assessing and coordinating for services such as home health aides, social work, chaplain, pre-bereavement and volunteers.
  • Maintaining records of patient care plans, physician orders, medication administration, and other information about a patient’s condition.
  • Providing counseling and support to patients and families during difficult times in their lives
  • Recommending appropriate treatments and equipment for patients based on their condition and available resources
  • Ensuring adequate symptom control, including referral to acute care and medication supply.

Location: ThedaCare Hospice covers 13 different counties split up into 5 different teams of Case Managers.

This position requires a weekend rotation of every 8 weeks and 1 holiday per year.
