The Care Coordinator Team Lead provides clinical leadership and mentoring to RN Care Coordinators and Social Workers on their designated team.

The team lead is available to facilitate patient care, problem-solving, and appropriate use and stewardship of resources.

The team lead also serves as a contributor to the overall clinical, quality, and financial performance of the Department.

Licensure, Certification, or Registration Requirements for Hire:
REGISTERED NURSE: Current RN licensure in Virginia or eligible (NURSING ONLY)

BEHAVIORAL HEALTH: Current LCSW licensure in Virginia required.

CLINICAL SOCIAL WORK: N/A


Licensure, Certification, or Registration Requirements for continued employment:

REGISTERED NURSE: Current RN licensure in Virginia or eligible (NURSING ONLY)

BEHAVIORAL HEALTH: Current LCSW licensure in Virginia required.

Experience REQUIRED:
Minimum of five (5) years of clinical experience

Experience PREFERRED:
Previous mentoring and/or clinical leadership experience

Education/training REQUIRED:
REGISTERED NURSE: Baccalaureate Degree in Nursing from an accredited School of Nursing or
BEHAVIORAL HEALTH/CLINICAL SOCIAL WORK: Master’s Degree in Social Work from a Council on Social Work Education (CSWE) accredited program

Education/training PREFERRED:
REGISTERED NURSE: Master’s Degree in Nursing from an accredited School of Nursing preferred or
BEHAVIORAL HEALTH/CLINICAL SOCIAL WORK: Licensed Clinical Social Worker (LCSW) in Virginia preferred

Independent action(s) required:
Clinical decision-making in collaboration with physicians and daily prioritization of activities.
