Pay Range

$21.85 - $32.80

MAJOR RESPONSIBILITIES

  • Manage the intake of social needs referrals and requests for Social Health team services.
  • Establish referral communication strategies for Social Health team members.
  • Assist with internal and community-based organization outreach and relationship building work.
  • Provide training on the Find Help platform.
  • Assist with organizing Find Help data and updating user groups.
  • Help with Find Help technical management for listed Social Health team programs.
  • Manage the CRH group email and respond to inquiries from internal teammates and external community partners.
  • Communicate with Community Health Worker team members to facilitate client care.
  • Identify resources for clients to overcome barriers to care and facilitate access to community resources, including locating housing, food, clothing, prenatal classes, parenting, and providers to teach life skills, and relevant mental health services.
  • Remain aware of current services offered by service providers, such as mental health, housing, and employment assistance.
  • Works to reduce cultural and socio-economic barriers between community members and institutions.

MINIMUM EDUCATION AND EXPERIENCE REQUIRED

Level of Education:  High School Diploma or GED required.

MINIMUM KNOWLEDGE, SKILLS AND ABILITIES (KSA)

  • Self-directed with excellent time management and detail orientation.
  • Ability to work under pressure in a fast-paced healthcare environment with tight deadlines.
  • Adaptability based on changing needs of the organization.
  • Knowledgeable of social health, social determinants of health, community health, evaluation, and health literacy principles.
  • Ability to engage community health worker teams and other care teams.
  • Ability to navigate Epic or another Electronic Medical Record (EMR).
  • Ability to navigate social care networks and platforms.
  • Strong interpersonal skills, good communication and literacy skills, and demonstrates the ability to develop rapport with community health workers and care teams.

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

  • This position requires travel, therefore, will be exposed to weather and road conditions.
  • Operates all equipment necessary to perform the job.
  • Exposed to a normal office environment.
  • Light to moderate walking and standing, occasional lifting of 20 -30 pounds for community events.
