Job Description:

Licensed Practical Nurse (LPN) - Join the Elara Caring Team!

As an LPN, you’ll be a crucial part of our elite Hospice of healthcare professionals, providing personalized hospice care to clients in their homes under the supervision of a Registered Nurse.

Schedule: Monday through Friday, 8:00a to 5:00p

Coverage Area: Methuen to Groveland to Salisbury

Summary of Key Responsibilities:

  • Patient Care: Provide skilled nursing care according to the Plan of Care and physician's orders, under RN supervision.

  • Care Coordination: Communicate patient findings and changes to the care team, including the RN Case Manager and physician.

  • Patient Safety: Implement safety plans using available resources.

  • Education: Teach patients self-care techniques and manage medications.

  • Documentation: Keep accurate records of patient care and progress.

  • Collaboration: Work with the RN Case Manager for support and guidance.

  • Patient Advocacy: Ensure respectful care to prevent avoidable hospitalizations.

  • Compliance: Follow HIPAA and safety protocols, adhering to Elara Caring's mission.

Requirements:

  • Current LPN license as required by state

  • 1 year of experience in an acute care or equivalent setting

  • CPR certification with American Heart Association or America Red Cross

  • Positive attitude

  • Dedication to quality patient care

  • Strong communication skills

  • Reliable transportation to perform job duties
