Crossroads is a leading addiction treatment provider of outpatient medication-assisted treatment (MAT). We treat patients with opioid use disorder (OUD) using medications such as methadone and suboxone/ buprenorphine. We pride ourselves in supporting our patients’ medical and personal recoveries from substance use disorder. Day in the Life of a Nurse Practitioner

• Evaluate and manage patients with opioid use disorder in our outpatient clinic settings

• Prescribe appropriate medications and treatments as indicated by patient assessment

• Complete medical documentation per Crossroads standards

• Collaborate with clinic staff to provide holistic care to patients, including referrals to other medical providers and community services

Schedule & Locations

• Louisville: OTP and Louisville OBOT

• Target weekly hours: 19

• Target schedule:

• Mondays OTP 5am-11am

• Wednesdays OBOT 9am-12

• Thursdays OTP 5am-11am

• Saturday OBOT 8am-12

• Flexible on days of the week for OTP but must be able to start at 5am.

Education and Licensure Requirements

• Certified as a Nurse Practitioner in the state you are applying to work in

• DEA licensure in the state you are applying to work in

• Additional education or certification may be required depending on state of practice
