Corewell Health West is seeking to hire a board certified/board eligible Family Medicine or Internal Medicine/Pediatrics physician to join our Community Medicine Clinic (CMC) located in downtown Grand Rapids, MI.  CMC provides full primary care to patients regardless of resources or ability to pay.

Providers at CMC provide care in a team-based model with social workers, pharmacists, care managers, a population health nurse, and other valued members of the care team.  Many of the patients served at CMC are experiencing homelessness, mental health disorders, and addiction.  With close proximity and strong relationship with the Corewell Health Addictions Clinic, patients receive comprehensive care for both their physical and mental health needs with respect and kindness.    

The ideal physician candidate is one who has a passion for promoting justice in medicine for minority and disadvantaged patients.  CMC works to eliminate disparities in health by promoting a healthy lifestyle among an underserved population.

Essential Functions

  • Assesses and monitors the medical needs of patients/significant others.
  • Provides quality medical services to patients in a cost effective, appropriate manner and ensures coverage of case load at all times.
  • Provides input into the development of policies and procedures as they related to medical practice and quality patient care.
  • Provides oversight of Physician Assistants and Nurse Practitioners and others providing delegated services to patients.
  • Evaluates patients'/significant others'/staff educational needs and develops and implements appropriate teaching strategies or delegates to appropriate resources.
  • Documents in a manner consistent with regulatory standards.
  • Communicates with other members of the health care team in order to ensure continuity of care and coordination of services. Acts as a patient/family advocate.
  • Participates in ongoing process improvement activities through the development/application of clinical paths, process improvement and other related activities.

    Actively works with Corewell Health to meet financial and patient satisfaction goals.

Qualifications

  • Required bachelor’s degree.
  • LIC-Physician (MD) - STATE_MI State of Michigan Upon Hire required Or
  • LIC-Osteopathic Physician (DO) - STATE_MI State of Michigan Upon Hire required.
  • CRT-DEA Registration - STATE_MI State of Michigan Upon Hire required.
  • CRT-Basic Life Support (BLS) - AHA American Heart Association 120 Days required Or
  • CRT-Basic Life Support (BLS) - ARC American Red Cross 120 Days required.
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association If required by specialty or stress test performed 120 Days required.
