Job Description 

In this community nursing role, as a Part-Time Night-Shift Registered Nurse (RN) you will have scheduled visits between the hours of 10pm - 8am to care for patients in the Waterloo Region. You will work with patients of all ages with differing complexity of care needs and play a key role in helping patients remain comfortable in their homes for as long as possible.

What you’ll do: 

  • Using appropriate assessment tools/techniques to assess patients’ individualized needs 
  • Executing and continuously monitoring care plans  
  • Engaging patients using a collaborative approach by building rapport and trust 
  • Maintaining and updating patient health records related to encounters, interventions and plans 
  • Ensure care is provided with established nursing standards and principles, physicians’ orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents / patients
  • Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the ‘big picture’ and generate creative solutions
  • Ability to work alone and with others while prioritizing tasks to handle the most urgent first

What you bring: 

  • Registration with the CNO as a Registered Nurse in good standing 
  • Valid driver’s license and access to a registered vehicle is required 
  • Palliative care experience is an asset
  • Post-graduate education in Palliative care is mandatory
  • Experience in delivering community care is an asset 
  • Reliability and flexibility  
  • Willingness to travel to and from patient residences within a designated area 
  • Passion for making a difference in your community 
  • Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanour
