Primary Duties & Responsibilities:
Conducts a comprehensive sensitive assessment with the client and family/support network as appropriate, in order to develop a plan of care to meet identified needs.
Arranges and coordinates service referrals for the client/family as needed; provides ongoing monitoring of the activities implemented to carry out the plan of care. Refers clients/families to public benefits programs as eligible.
Provides regular contact with enrolled clients and documents all contacts and service coordination activities in client records as required by program guidelines.
Educates clients/families about the basic medical and psychosocial aspects of disease in order to optimize adherence to care.
Provides proactive follow up, tracking symptoms and treatment responses.
Adheres to all applicable state laws and the department’s rules and regulations regarding programs.
Stays abreast of new resources, programs and developments. Participates in continuous training and education, including attendance at staff meeting and community meetings as assigned.
