Job Summary:


The Director of Population Health Strategy serves as a key partner with leadership to shape and execute the overall mission and vision of the Wellstar Population Health, Care Coordination and Wellstar Clinical Partners
Divisions by taking accountability and responsibility for the following:
Develop the business and analytics strategies from planning to execution for supported areas.
Mentor and develop the analyst team to support innovation, optimize data-driven decision making and provide key insights into operational and population-based questions.
Ensure that the clinical teams have the tools and knowledge to effectively manage the health of the targeted populations by delivering timely, actionable and accurate information.
Develop a cohesive reporting strategy to support the data, analytics and scorecards functions for all value based programs to extract critical insights for operational excellence.
Mine and analyze data from internal databases and external sources to:
o Drive optimization and improvement of product development, marketing techniques and business strategies
o Evaluate the effectiveness and profitability of programs and promotions
o Forecast future trends and project market potential
Serve as the primary partner and liaison with the Data and Analytics Hub and Spokes for Population Health and
Care Coordination to align and partner on the following:
o Identify opportunities to leverage internal and external data to drive goal achievement
o Implement and monitor outcomes
o Evaluate the effectiveness and accuracy of new data sources and data gathering techniques
o Develop custom data models and algorithms
o Use predictive modeling to increase and optimize and target outcomes
Work directly with the AVP Population Management to build and manage the budget for Population Health
Management
The Director will be responsible for directly supporting the analytics and strategic needs of the following
programs:
WellStar Clinical Partners (e.g. Administration, Board and Committees, Quality Program)
Value Based Care Contracts
o Wellstar Medicare FFS ACO
o Employee Health Plan
o Cigna
o Humana Commercial
o Humana MA
Population Health Case Management
Preventative Health (e.g. Colorectal Cancer Screening, Lung Cancer Screening, Incidental Lung Nodule)
Diabetes Service Line and System Diabetes value stream
System Inpatient Care Coordination and Utilization Management
Post-Acute Care Network
Community Health and Center for Health Equity

Core Responsibilities and Essential Functions:


Data Integration, Systems Management, Analytics, Insights & Visualization
a. Lead Population Health and Care Coordination analyst team in design, development and
implementation of analytics and visualizations for key organizational information needs by providing
guidance to the analytics teams on the proper analytics tool use based on business needs and
aligning to strategic priorities
b. Serve as an organizational Subject Matter Expert (SME) to the organization on best practices for
Analytics and Visualization regarding tools and practice and advise direct and indirect analytical
teams on best practices regarding population health related data
c. Continually evaluate the data sources, integration techniques, implementation process,
visualization tools and monitoring strategies used to deliver timely, relevant and actionable
information to the Population Health, Care Coordination and Wellstar Clinical Partner teams
d. Design and partner on data aggregation strategies to blend information from multiple disparate
systems to deliver curated data models to meet the needs of the supported programs (e.g. Epic
health record, claims data from payers, claims data from WCP Community Partners). this includes but is not limited to implementation and ongoing management of any current and future advanced data platforms which includes:
-Finalizing platform configuration decisions by aggregating feedback and translating into requirements
-Overseeing the implementation of data connectors inclusive of initial and ongoing validation of data pipelines for accuracy, reliability, and quality.
-Developing user management workflows and standards
-Leading internal strategy for user training programs
e. Serve as primary partner and liaison between the Data and Analytics Hub and Spokes to ensure
that the technological needs of the division are satisfied and the appropriate alignment to system
data governance standards and policies
f. Engage with the Data and Analytics Hub leadership to advance the objectives of data
transformation within the organization
g. Manage the integration of the claims and eligibility files directly from payers for the Wellstar
Clinical Partners Value Based Care contracts for use in reporting financial, utilization and quality
performance
h. Manage the security and access policies for third-party data warehousing and analytics tools in
partnership with Compliance
Data Mangement and Engineering: Directing and actively participating in the automation of data acquisition, transformation, and loading processes, integrating this data into external platforms as well as internal data models and systems for use in advanced data models.
Integration of EHR and Quality Data
Oversee, validate and support the integration of over 100 EHR and quality data submissions, ensuring compliance with new CMS CQM reporting requirements.
Strategic Leadership & Business Development
a. Develop and maintain a comprehensive population health analytics strategic plan that aligns to
system plans for data transformation, clinical integration and quality improvement across sites,
populations and providers and for WCP's ever growing value-based payer contracts, including meeting the data integration and reporting needs of Wellstar's Health Plan.
b. Synthesizes information and data to develop a deep understanding of population health and
clinically integrated network landscape to advance innovation at Wellstar and beyond
c. Develop KPIs, strategies and tactics in partnership with subject matter experts and reports
progress of Population Health initiatives and programs to executive and board leadership
d. Provide the strategic leadership, collaborative coordination, creative problem solving and
administrative authority necessary to manage and advance new and existing opportunities
e. Manage relationships for the various vendors partnering with Population Health Management
Personnel & Business Unit Management
a. Manage and mentor the analytical, strategic and project management resources within Population
Health Management
b. Curate resources for individualized and team development to promote data literacy, strategic
integration, process improvement and technological advancement in alignment with division and
system goals and objectives
c. Partner with Population Health Management leadership to build and manage annual budget
planning and off-cycle budget requests
d. Develop key metrics to track efficiency and effectiveness of resources and programs managed
through the Population Health and Care Coordination programs

Required Minimum Education:

  • Masters Business Administration/Management or Masters Computer and Information Science

    Required Minimum Experience:


    Minimum 8 years experience in healthcare provider setting, preferably with exposure to
    both the clinical and revenue cycle areas Required
    Minimum 4 years leadership experience in managing multiple, cross-function teams and
    projects and influencing senior level leadership Required
    Extensive experience in adopting and utilizing data and analytics tool and
    platforms to build program strategies Required
    Experience working with third-party consultants and technology firms to
    support strategic objectives and operations Required


    Required Minimum Skills:


    Extensive knowledge and understanding of the changing healthcare environment, especially
    as it relates to population health and value-based care arrangements
    Comprehensive understanding of enterprise information systems and relevant external data
    sources with the ability to develop reporting packages that promote data driven decision making
    Ability to partner with system stakeholders through influence
    Ability to manage relationships with third-party vendors to ensure obligations are met and
    expected value is delivered in the arrangement
    Advanced skills in Microsoft Office 365 products, especially Excel, Power Platform and
    PowerPoint
    Advanced SQL, Visual Basic and Tableau experience preferred
    Familiar with machine learning, AI, predictive models and other data science concepts
