Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not a cure, and on quality of life, not duration. As RN this team, you will collaborate with physicians, RNs, Social Workers, Home Health Aides and volunteers to support patients wherever they call home. This position offers a flexible schedule and autonomy to work independently, allowing you to maintain a healthy work-life balance.
This role will focus on the following counties: East Cuyahoga, Summit, Portage, Geauga, Lake and Stark
A caregiver in this role works hours of 11:00am to 11:30pm and every other weekend. 1 winter and 1 summer holiday is required and will rotate every 3 years.
A caregiver who excels in this role will:
Provide supervision, assessment, planning and direct patient care to hospice patients.
Perform accurate and complete admissions assessments and re-assessments.
Assess patients for safety risks, abuse and neglect.
Assess, document and manage patient’s pain.
Prioritize and document patient problems.
Administer medications and treatments safely and accurately with a focus on patient comfort.
Coordinate care for managing a caseload of patients.
Document changes in patient condition and effectiveness and timeliness of care/interventions.
Evaluate, update and revise plans of care.
Assess educational needs of patients, caregivers and family while considering cultural issues and other special challenges.
Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.
Minimum qualifications for the ideal future caregiver include:
Graduate from an accredited School of Professional Nursing
Current licensure as a RN in the State of Ohio
Basic Life Support (BLS) certification through the American Heart Association (AHA)
Valid Driver’s License (VDL) and proof of $100,000/$300,000 automobile insurance coverage
Minimum 1 year current clinical experience as a registered nurse.
Minimum 1 year experience in an acute medical-surgical or home care setting.
Preferred qualifications for the ideal future caregiver include:
Bachelors of Science in Nursing (BSN)
Hospice and Palliative Care Nurse Certification
