JOB SUMMARY 

As the Assistant Director of Clinical Services, your role is pivotal in orchestrating the day-to-day clinical operations for hospice and supporting the clinical functions of home health services. Working collaboratively with the Director of Nursing, you'll contribute to the development and implementation of policies, procedures, and protocols, ensuring a seamless and high-quality patient care experience. Your leadership will be crucial in providing guidance to clinical staff, including supervising Hospice Nurses, as well as actively participating in recruitment, onboarding, and conducting performance evaluations. 

In addition to overseeing compliance with regulatory standards, your responsibilities extend to developing and implementing staff training programs aligned with evidence-based standards. Serving as a liaison between nursing staff and other departments, you'll manage and resolve issues related to patient care, staff performance, and operational efficiency. Active participation in budget development, effective communication with healthcare professionals and community partners, and staying informed about industry trends underline your commitment to excellence. Your role also encompasses community engagement, representation in various meetings and events, and collaboration on infection control measures and safety protocols, reinforcing the organization's dedication to providing exceptional clinical services. 

DUTIES & RESPONSIBILITIES 

• Oversee and coordinate day-to-day clinical operations for hospice and supports the clinical operations for home health services. 

• Collaborate with the Director of Nursing to develop and implement policies, procedures, and protocols for patient care. 

• Ensure compliance with regulatory requirements and accreditation standards in both hospice and home health settings. 

• Provide leadership and support to clinical staff ensuring a high standard of clinical care. Supervises Hospice Nurses. 

• Assist in recruiting, hiring, and onboarding nursing personnel for hospice and home health. 

• Conduct regular performance evaluations and provide ongoing feedback to hospice clinical staff  

• Develop and implement staff training and education programs to enhance clinical skills and knowledge. 

• Collaborate with the interdisciplinary team to assess and address patient needs comprehensively. 

• Monitor and evaluate patient care plans to align with best practices and evidence-based standards. 

• Assist in implementing quality improvement initiatives for enhanced patient outcomes. 

• Act as a liaison between nursing staff and other departments, fostering effective communication. 

• Manage and resolve issues related to patient care, staff performance, and operational efficiency. 

• Participate in budget development and maintenance for nursing services. 

• Ensure effective communication and coordination with healthcare professionals and community partners. 

• Stay informed about industry trends, regulations, and best practices in hospice and home health care. 

• Collaborate to maintain relationships with community partners, including hospitals and assisted living facilities. 

• Represent the organization in meetings, conferences, and community events. 

• Assist in developing and implementing infection control measures and safety protocols. 

• Participate in developing and implementing disaster preparedness and emergency response plans. 

• Foster a positive work environment promoting teamwork, professionalism, and commitment to the organization's mission and values. 

• Attends, participates, and leads Hospice IDG meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. 

• Conducts monthly and quarterly record reviews and communicates findings and recommendations to Clinical Director and hospice personnel. 

• Attends leadership, QAPI, clinical, marketing, and other meetings as appropriate. 

• Participates in the AOC on-call rotation. 

• Acts as DCS designees' backup for the state licensure for both home health and hospice. 

• Manages caseloads to ensure work is evenly distributed within the time, limiting overtime, or underproductive staff. Works on staffing solutions. 

• Stay informed about industry trends, regulations, and best practices in hospice and home health care. 

• Collaborate with the Director of Nursing to develop and maintain relationships with community partners, including hospitals, assisted living facilities, and other healthcare providers. 

• Represent the organization in meetings, conferences, and community events as needed. 

• Assist in the development and implementation of infection control measures and other safety protocols. 

• Participate in the development and implementation of disaster preparedness and emergency response plans.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities) 

• Must be a registered nurse  with current licensure to practice in the State. 

• Bachelor’s degree in Nursing from an accredited program by the National League for Nursing.  Master’s degree in hospice preferred. 

• CHPN preferred  

• Two years of management experience in a related health care organization. 

• Demonstrated ability to supervise and direct professional administrative personnel. 

• Ability to market and deal tactfully with customers and the community. 

• Must be a licensed driver with an automobile that is insured in accordance with state and/or Organization requirements and is in good working order. 

• Has excellent observation, verbal and written communication skills. 

• Knowledge of business management, state and federal governmental regulations.
