Physical Therapist – $5,000 Sign-On Bonus

What You’ll Do

  • Promote Elara Caring’s mission, philosophy, and policies to ensure exceptional patient care
  • Provide direct patient care, including evaluations, treatment planning, and outcome assessments
  • Design and implement individualized plans of care tailored to each patient’s needs
  • Collaborate with interdisciplinary team members to ensure optimal outcomes
  • Direct and support therapy assistants and staff to achieve patient goals
  • Maintain accurate and timely documentation, including evaluations, treatment plans, and physician orders
  • Serve as a clinical resource and promote physical therapy within the community
  • Work in a physically demanding, fast-paced home-care environment
  • Perform lifting, transferring, and positioning of patients as required
  • May work irregular hours or participate in call rotation, if applicable

What We’re Looking For

  • Graduate of an APTA-accredited Physical Therapy program
  • Current, unrestricted Physical Therapist license in the state of practice
  • Minimum of one (1) year of experience in a healthcare setting
  • Willingness and ability to travel up to 50% within the assigned coverage area
  • Dependable vehicle, valid driver’s license, and current auto insurance
  • Ability to lift, push, pull, and support up to 50 pounds

You will report to the Branch Director, Clinical Manager, Clinical Supervisor, or designee.
