The Cleveland Clinic Florida Care Management team is made up of health care professionals dedicated to helping patients stay on track with their health goals. As an Assistant Nurse Manager in Care Management, you will assist with day-to-day operations, utilize clinical expertise and understanding of care management and Medicare regulations, contribute to the goals of cost containment and quality care and provide safe, appropriate transitions of care.
The ideal future caregiver is someone who:
Has a bachelor's of science in nursing (BSN).
Is an RN with care management certification.
Has at least two years of experience in care management or transitional care planning.
Has at least three years of experience in hospital-based nursing.
