Job Title: Clinical Resource Nurse

Department: 2600 - Telemetry Med-Surg

Location: Unity Hospital

Hours Per Week: Full Time - 40 hours

Schedule: Monday through Friday, rotating 8-hour shifts

SUMMARY:

An experienced registered nurse who is responsible for assisting, guiding, coaching, promoting, and executing quality care delivery throughout the inpatient acute care units. Responsible to round and assist staff nurses in improving quality care by assisting in the provision of direct care as needed. Assistance of teaching staff and patients to enhance a patient-centered care model and the patient’s individual plan of care. Assist in the assessment and completion of required core measures. Serves as a role model and staff nurse support and resource for excellence in clinical care and the overall patient experience. Provides superior patient/family customer service to align with the overall expectation of a positive patient experience.

Telemetry Med-Surg – 2600

2600 is a stroke certified, telemetry medical surgical unit. This is a 27 bed unit that cares for cardiac patients on telemetry monitoring. Diagnoses on this unit are primarily cardiac in nature, but the unit also cares for patients with a diverse variety of general medical illnesses. Because of this, our nurses develop a strong set of skills and knowledge that they can use to grow their career throughout the acute care environment. ​

RESPONSIBILITIES:

  • Conducts initial and ongoing patient assessments using appropriate physical, psychological, and developmental data; applies sound clinical judgment in identifying and addressing priority needs.
  • Collaborates with the multidisciplinary team to develop, implement, and revise individualized plans of care, ensuring documentation and communication reflect patient-specific goals and outcomes.
  • Acts as a clinical resource and mentor by rounding on units, assisting staff nurses with complex care, supporting admissions and discharges, and promoting safe, timely patient throughput.
  • Enhances the patient and family experience by maintaining effective, compassionate communication and ensuring education is tailored to patient needs and abilities.
  • Promotes excellence in nursing practice through adherence to organizational policies, active participation in quality improvement initiatives, and identification of opportunities for evidence-based practice.
  • Demonstrates adaptability and leadership by supporting staffing needs, guiding colleagues, managing stressors constructively, and collaborating on solutions to unit-level challenges.
  • Maintains professional development through continuing education, completion of mandatory in-services, and ongoing compliance with regulatory standards and competencies.

REQUIRED QUALIFICATIONS:

  • Diploma or Associate’s Degree in Nursing
  • Registered Nurse license in New York State.
  • Basic Life Support (BLS) certification.
  • 3-5 years of acute care experience.

PREFERRED QUALIFICATIONS:

  • Bachelor’s degree in Nursing
  • Excellent verbal and written communication skills
  • Demonstrated ability to effectively teach
  • Demonstrated ability to foster a collaborative, interdisciplinary environment that supports quality care/service and staff empowerment
  • Self-directed individual with high organizational and priority-setting abilities
