Summary:

Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients ensuring that safety, best practice, and high quality standards of care are maintained across the continuum.

Responsibilities:

Coordinates a wide range of activities and education to support the patient population by utilizing a variety of communication methods including but not limited to telehealth, face-to-face, text, telephone, or e-mail.Works closely with physicians and the multidisciplinary care management teams. Facilitates care coordination, and documents efforts.Extract data from medical record sources to gather information and perform outreach. Communicates details to the appropriate team, providers, and/or supervisors.Utilizes the appropriate reports to prioritize outreach to ensure care coordination.May assist with obtaining authorizations and scheduling where appropriate.

Requires critical thinking skills, effective communications skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Knowledgeable in Excel, Word and PowerPoint and able to use electronic medical records such as EPIC. Required Work Experience: One (1) year of experience in a healthcare setting.
