Job Summary: The Resident Care Coordinator is a shift leadership position which oversees the care of Atherton Place Personal Care Residents from a Resident Centered Approach by overseeing the day to day resident care, medication assistance/administration, medication reconciliation, and resident care plans and files. The coordinator will assist the supervisor and manager to assure that the facility is in compliance with all state rules and regulations. Will coordinate care with doctors, family, residents, and outside agencies to maintain the best possible level of health for each resident. Will act as the Designated Responsible Person in the absence of Supervisor or Manager. Will assist personal care supervisor with scheduling, development, and supervising of staff. Core Responsibilities and Essential Functions: Oversee daily health and safety of resident population a. Monitor medication assistance/administration process b. Administer medication in memory care unit. c. Assist in medication reconciliation of new residents and daily med changes, month check-in of resident meds. d. Coordinate care with doctors, families, outside agencies to maintain optimum health level. e. Monitor socialization/activities/behaviors to improve quality of life Assessment of resident population. a) Oversee any resident issues and make sure apppropriate care is initiated and follow up is done until issue is resolved. b) Monitor residents who self-medicate for appropriateness. c) Monitor and assist with update of resident assessments quarterly and after any change in status. d) Monitor mental status assessments quarterly. e) Interact with families concerning resident care as needed. f) Assist manager/supervisor with assessment of potential new residents/residents returning from hosp, rehab. State Readiness a) Provide monthly reports of resident population. b) Monitor resident records for accuracy. c) Assist with training of new staff and yearly educational updates. Serve as Designated Responsible Person a) Act as shift leader and contact person for State Surveyor in absence of the manager/supervisor. b) Be available to answer questions and supply resident and employee files to State surveyor in absence of the manager. c) Ensure that all procedures are followed in the event of an emergency or disaster situation in the absence of the manager. d) Help to find coverage for a staff call in and/or make manager/supervisor aware. e) Will research and attempt to solve any team issues that occur during shift. Required Minimum Education:
  • High School Diploma General or GED General or Other / Certificate Nursing
Required Minimum License(s) and Certification(s): All certifications are required upon hire unless otherwise stated.
  • Lic Practical Nurse
  • Basic Life Support or BLS - Instructor
Additional License(s) and Certification(s): Required Minimum Experience: 2-4 years experience in working with seniors in a long-term care setting Preferred Experience with memory impaired seniors is strongly Preferred Experience with State Survey Readiness Preferred Required Minimum Skills: Good customer service and communication skills required; demonstrated courteous and positive attitude; must be able to read and write; ability to relate well with staff members, residents, and families; current CPR or willingness to obtain instruction. Basic computer skills with Window based environment for participation in training and resident documentation. Organizational skills and attention to details are essential. Demonstrates good judgement, problem solving and decision making skills.
