Summary:

Performs/oversees daily operations of the Youth Outreach programs and services.

Responsibilities:

Develops and maintains daily activity schedules.

Provides case management and clinical services for clients and their families.

Evaluates and assesses the needs and strengths of clients and families.

Facilitates services related to the CYS program.

Provides case management, care coordination, group counseling to clients and families enrolled in program.

Performs evaluation and assessment of client and family needs Provides case management, care coordination, group counseling and informal counseling for clients and families.

Completes surveys and outcome tools with clients and families per grant requirements and reports outcome data to program team leader.

Transports clients and families to appointments and community resources.

Must be able to work in a stressful environment and take appropriate action. Must be able to handle a heavy case load and multi-task. Must be able to work a flexible schedule. Required Work Experience: No experience needed. Other Information: Must have Valid Florida driver's license, be cleared by legal to be added to Memorial insurance and have personal vehicle available during work hours as position requires extensive travel throughout Broward County.Additional Education Info: Bachelor's degree or Master's degree based on grant specifications.Additional Credential Info: New hires or employees new to the position have six (6) months to obtain the Basic Life Saving (BLS) Certification.
