JOB DESCRIPTION:

SUMMARY OF POSITION

The Veteran Care Navigator (VCN) is responsible for in-person veteran peer support, assisting veterans and members of the armed services, facilitating access and transitions in care.  The VCN connects with veteran organizations, educating the community on services available through CCBHC and facilitating referrals into care.  This position will support the development of the Certified Community Behavioral Health Clinic (CCBHC) to enhance and advance CCBHC requirements.  Ensures clients are receiving care as necessary to meet CCBHC core service obligations and in compliance with standards, regulations and laws, maintenance of appropriate documentation, etc.  Works collaboratively with CCBHC clinical teams, ensuring individuals served are engaged with treatment and other resources.  Consults with CCBHC leadership, as well as resources within the community to ensure services are provided are engaging and impactful for clients.  Cultivates a safe and supportive network in conjunction with provision of recovery education and advocacy using, trauma-informed, person-centered, and share-decision making models.

ESSENTIAL DUTIES & RESPONSIBILITIES

  • Provides services meeting quality standards and in compliance with various regulatory bodies as well as key performance indicators as required.
  • Represents Centerstone to internal and external stakeholders, funders, and community partners; creates relationships to successfully develop CCBHC programming; provides community education and consultation as needed.
  • Supports operations that allow Centerstone to meet all deliverables in the timeframes identified within the state contract for CCBHC to ensure fidelity.
  • Participates in process reviews or in set up of new program processes as needed to ensure compliance with appropriate rules and to ensure quality clinical documentation.
  • Contributes to the management of resource and provider network database.
  • Supports effective service integration and/or coordination across intensive, community-based behavioral health care settings, including with local Veterans Affairs (VA) health providers.
  • Provides targeted case management and referrals for outpatient counseling services with providers using shared-decision making model to ensure patient care meets individualized needs and program compliance standards for CCBHC.
  • Encourages eligible patients and their families to utilize Centerstone’s programs while also identifying and addressing barriers to engagement using brief, solution-focused emotional support techniques.
  • Demonstrates an ability to assess risk and deliver safety management interventions, including danger to self and others, to ensure the safety and stabilization of clients and to make appropriate referrals/linkages when clinically indicated. Does not leave crisis situation until client is in a safe environment or crisis is resolved.

  • Skill in responsible team collaboration, time management and prioritizing tasks.
  • Ability to problem solve complex issues and deliver effective decision making.
  • Knowledge of community resources specifically related to Veteran Serving Organizations.
  • Ability to effectively communicate via written, verbal, in-person, and virtual methods.
  • Ability to attend in person meetings

QUALIFICATIONS

Education Level

Bachelor's Degree in human services field or 5 years of experience in the behavioral health field

Years of Experience

Military Veteran status required

Certification/Licensure

State Certified Veteran Support Specialist, or, willing and able to obtain the certification upon hire.

$18.20--$25.50
