Title: Senior Analyst, Patient Retention

Role Description

The Senior Analyst, Patient Retention will work closely with the Patient Engagement team, as well as the Growth and Population Health teams, at Oak Street to drive key initiatives supporting the patient journey at Oak Street Health. In particular, this role will focus on improving patient retention through coordinating outreach across engagement, population health and clinical teams to identify highest priority needs for each patient. The role will also include owning day to day workflows for the patient segmentation effort which identifies the lowest common denominator for each patient that is preventing them from completing their annual health review visit.  Success for these initiatives will be measured through completion of health review visits for tenured patients. 

 

This role will consist of small and large group training, exposure to senior leadership, and the opportunity for quick career progression and collaboration across many business functions. The Senior Analyst will be responsible for driving outcomes, managing relationships, and creating insights to improve both strategic and operational workflows on the Patient Engagement team.

Core Responsibilities

  • Drive strategic analyses, develop data-driven insights & own reporting 

    • Analyze data to inform strategic decision-making related to patient retention 

    • Build and pull reports on performance metrics related to patient retention

    • Complete analyses to identify areas of progress and opportunity

    • Communicate insights with local leaders and organizational leaders

  • Drive patient retention at Oak Street through implementation of Patient Engagement and Retention workflows 

    • Manage relationship with key partners across provider services, Salesforce and patient comms and marketing

    • Own proactive workflows related to patient communications and panel reassignment 

  • Contribute to key organizational retention goals through implementing initiatives to meet annual patient annual wellness visit completion goals

    • Identify key factors that influence annual wellness visit completion and identify patient populations for targeted outreach

    • Report on success of different engagement strategies for achieving annual wellness visit completion goals 

  • Project Management & Programmatic Ownership

    • Project manager for scope of ongoing initiatives and activities related to patient segmentation

    • Creation of metrics and reports to monitor success and identify opportunities for improvement across the implementation of new initiatives 

  • Other duties as assigned  

What are we looking for?

  • Minimum 2-4 years of experience

  • High level of organization and detail orientation

  • Experience collaborating on projects across multiple departments

  • Proficient with Microsoft Excel or other standard analysis tools (SQL, Alteryx, etc.) 

  • Strong intellectual curiosity and problem-solving ability

  • Ability to navigate leadership and operational complexities

  • Passion for excellence, willing to go the extra mile to achieve objectives and ensure consistently high quality work product

  • Quick learner comfortable with ambiguity

  • Strong accountability and discipline against timelines and deliverables

  • Collaborative working style, able to work cross functionally

  • Bachelor’s degree required 

  • US work authorization

Pay Range

The typical pay range for this role is:

$46,988.00 - $112,200.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
