Position Summary

The Care Management Associate Director is responsible for overseeing the operations, development, and implementation of care management programs and strategies to improve clinical outcomes and reduce healthcare costs. This role involves overseeing the operations and strategic direction of care management within an ACO, collaborating with healthcare providers, and analyzing data to identify opportunities for cost savings and quality improvement.

This is a remote opportunity, open to any US state, but the candidate would need to work hours that would be typical of either EST or CST.

 Responsibilities:

  • Collaborate with healthcare providers to ensure coordinated and efficient care delivery.
  • Analyze clinical and financial data to identify trends and opportunities for improvement.
  • Develop and monitor key performance indicators (KPIs) related to care management and affordability.
  • Implement strategies to improve patient outcomes and reduce hospital readmissions.
  • Ensure compliance with regulatory requirements and accreditation standards.
  • Provide training and support to care management staff.
  • Develop and maintain relationships with external partners, including insurance companies and healthcare providers.
  • Prepare and present reports on care management and affordability initiatives to senior leadership.
  • Develop and communicate a clear product vision and strategy that aligns with the organization’s goals and patient needs
  • Continuously monitor product performance and adjust strategies based on metrics, customer feedback, and market trends
  • Work closely with cross-functional teams, including design, engineering, and marketing, to ensure successful product development and delivery
  • Define and prioritize product features based on customer needs, business goals, and technical feasibility2.
  • Collaborate with stakeholders to create detailed product requirements and user stories
  • Coordinate with sales and marketing teams to prepare for product launch and ensure effective communication with customers
  • Monitor product performance post-launch and make necessary updates or improvements
  • Gather and analyze user feedback to continuously enhance the Care Management product


Required Qualifications

  • Bachelor’s degree in Nursing, Healthcare Administration, or a related field
  • Current RN license
  • 5+ years of experience in care management or clinical leadership roles.
  • Product management experience is required
  • Strong understanding of care management processes and healthcare systems.
  • Excellent communication and collaboration skills.
  • Ability to analyze data and make informed decisions
  • Strong understanding of healthcare regulations and cost management strategies.
  • Excellent leadership and organizational skills.
  • Proficiency in data analysis and healthcare management software.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with diverse stakeholders.
  • Experience with quality improvement initiatives and performance measurement.



Preferred Qualifications


Education

Bachelor’s degree in Nursing, Healthcare Administration, or a related field

Pay Range

The typical pay range for this role is:

$123,857.50 - $266,770.00


This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.  This position also includes an award target in the company’s equity award program.
